What is Hospice Care?
Hospice care is a comprehensive and holistic approach to care that is geared towards the quality of life for people and their family/caregivers who are experiencing an advanced, terminal illness.
Hospice is a patient-centered and family-oriented program that helps families/friends/caregivers to provide care for their loved one in the home, nursing home or care facility.
Hospice is not a resignation to hopelessness and helplessness but a means to face a life-limiting condition with hope of dignity and comfort.
Making the decision to place a loved one in a long term care facility can be very traumatic. Hospice can help both the patient and the family through this transition.
Hospice is covered by Medicare, Medicaid, and other insurance providers. No one is ever denied service based on inability to pay.

Together with you , your family/friends, your attending physician and our own team of doctors, nurses, social workers, hospice aides, Chaplain, Bereavement Specialist, volunteers and others, we will make the final journey meaningful, dignified and as comfortable as possible.
Modevie Hospice Level of Care
Routine Care
Intermittent home visits from members of our hospice care team focusing on symptom/pain management and support of the patient and family/responsible persons in the comfort of their home.
Continuous Care
When symptoms are severe and out of control, a nurse or home health aide may stay in the home for a minimum of 8 hours until the symptoms are managed successfully.
General In-Patient Care
When symptoms/pain require an advanced level of care, a short-term in-patient care in a nursing facility is available while the hospice team continues care and services.
Respite Care
Relief for the unpaid family caregivers from the challenges of caring for a loved one with advanced illness can be offered through a short-term inpatient care stay in a contracted nursing or assisted living facility.
